Your First Visit
In order to build a relationship of mutual respect and understanding, it is worthwhile for us to take time getting acquainted with one another.

We are interested in knowing you as a person and want to understand what is important to you. Therefore we will need to get to know you and your dental goals at our first meeting.

Please print and fill out all the forms at the right before your appointment to help make your first visit as productive as possible.

Financial Policy
Patients are expected to make financial arrangements at the time of treatment. Our office accepts most insurance coverage as well as MasterCard and VISA.

CareCredit® is a flexible payment program that lets you begin your treatment or procedure immediately - then pay for it over time with monthly payments. CareCredit is ideal for co-payments, deductibles, and treatment not covered by insurance.
